The court Successor by Merger to denied defendant’s initial fee waiver request because the reported Discover Bank v. gross monthly income of $5,707.14 substantially exceeded the Berenisse Ruiz reported monthly expenses of $2,739.44. Defendant’s FW-006 request states that the initial fee waiver application involved “temporarily higher” income. Defendant does not persuasively explain why defendant reported incorrect information on the initial fee waiver.

Defendant is ordered to file new FW-001 and FW-003 forms with accurate, updated financial information demonstrating eligibility for a fee waiver.
