(a)The articles of incorporation of a telephone cooperative must:

(1)state that the articles are executed under this chapter;

(2)be signed by each incorporator and acknowledged by at least two incorporators; and

(3)state:

(A)the name of the cooperative;

(B)the purpose for which the cooperative is formed;
