§ 604.152 — NOTICE OF CERTIFICATE RENEWAL

(a)Not later than the 30th day before the expiration date of a person's certificate, the advisory board shall mail a renewal notice to the person at the person's last known address.

(b)To renew a certificate, the certificate holder must:

(1)complete the renewal notice and return the notice with the renewal fee to the advisory board on or before the expiration date; and
