In addition to the information required by Section 3.005, the certificate of formation of a professional entity must state:
(1)the type of professional service to be provided by the professional entity as the purpose of the entity; and
(2)that the professional entity is a:
(A)professional association;
(B)professional corporation; or
(C)professional limited liability company.
