The notice of claim of lien must include:

(1)the name and address of the lien claimant;

(2)the name and address of the lien debtor;

(3)the location of the livestock for which the animal feed was provided;

(4)a statement that the payment of the reasonable or agreed charges is more than 30 days overdue;

(5)the amount that is overdue;
