§ 39.00146 — Case record face sheet

(1)As used in this section, the term:(a) “Multidisciplinary team” has the same meaning as provided in s. 39.4022(2).
(b)“Placement change” has the same meaning as provided in s. 39.4023(2).
(c)“School” has the same meaning as in s. 39.4023(2).
(d)“Sibling” has the same meaning as in s. 39.4024(2).
(2)The case record of every child under the supervision or in the custody of the department or the department's authorized agents, including community-based care lead agencies and their subcontracted providers, must include a face sheet containing relevant information about the child and his or her case, including at least all of the following:(a) General case information, including, but not limited to, all of the following:1. The child's name and date of birth. 2. The current county of residence and the county of residence at the time of the referral. 3. The reason for the referral and any family safety concerns. 4. The personal identifying information of the parents or legal custodians who had custody of the child at the time of the referral, including name, date of birth, and county of residence. 5. The date of removal from the home. 6.7=The name and contact information of the attorney or attorneys assigned to the case in all capacities, including the attorney or attorneys that represent the department and the parents, and the guardian ad litem.
