View on leginfo.ca.gov(a)The Attorney General or his or her delegate shall approve any claim form which shall be provided to a person aggrieved by the Travel Consumer Restitution Corporation to be submitted by a person aggrieved in order to obtain payment from the restitution fund. The claim form shall require the person aggrieved to provide the corporation with information which is sufficient to decide whether payment is to be made to that person. The information must include all of the following:
(1)The name, address, and telephone number of the person aggrieved.
(2)The date, form, and amount of each payment and evidence thereof.
