Appoint PD.

The Court deems the proceeding to be seeking a limited conservatorship. (Prob. Code, § 1800.3, subd. (d).) Notice to the Tri-Counties Regional Center is needed. An assessment from the Regional Center will also be needed.

The petition fails to check any orders requested at Paragraph 1. Are no orders being requested?

Paragraph 4.b and 4.c of the petition are not completed.

Petitioner states that the proposed conservatee will be unable to attend the hearing due to a medical inability. No affidavit or certificate of a licensed medical practitioner establishing the medical inability appears to be filed. (Prob. Code, § 1825, subd. (a)(2), (b).)

Paragraph 9.d is not completed. Paragraph 11 fails to state relationship of the persons listed to the proposed conservatee.

The Court Investigator’s Report identifies the maternal grandparents as Jose Alfredo Valencia and Rosario Valencia. These persons are not named at Paragraph 11 of the petition. They were also not sent the notice of hearing.

As of 5/18/26, the proof of service of citation has not been filed. The confirmation of viewing conservatorship video has not been filed.

The Court will continue the hearing for all of the foregoing issues to be addressed. Where appropriate, a verified supplement to the petition, as opposed to an amended petition, should be served and filed.
