REALTY, L.P., et al. The Court finds the following deficiencies in the Petition: 1) Paragraph 3b of MC-350 lists a trial date of 10/15/2025. The action has settled and there is no trial date scheduled. 2) Paragraph 8a of MC-350 provides that "[t]he claimant has recovered completely from the effects of the injuries described in item 6, and there are no permanent injuries." Petitioner should clarify that Plaintiff is deceased. 3) Petitioner has indicated that no defendant has offered to pay money to anyone other than the claimant in this action, thus, Paragraph 11b should remain blank. 4) Petitioner has listed the total amount of statutory or contractual liens as $25,258.95 in Paragraph 12a(5).
