The following must be submitted: Notice of Conservatee's Rights (Form GC-341) (Prob. Code, Sec. 1830(c)) Conservatee's Care Plan (Form GC-355/GC-356) (Prob. Code, Sec. 2351.2) The court further notes that a proposed order and proposed letters were never submitted to the court after the general conservatorship was granted on 11/20/2025. The temporary order and letters issued on 10/10/2025 have expired. The next Review of a Court Investigator's Report is already scheduled for 02/18/2027.
