Decree changing name is signed this date and electronically transmitted for filing.
Once the Court grants the Petition and signs the Decree Changing Name, a certified copy will need to be obtained from the Records Department, Room 218. A certified copy of the Decree will be needed to change the name on legal documents, including birth certificates, social security cards, and other government-issued identification such as passports or drivers licenses.
