ORDER CHANGING MINOR'S LAST NAMES CORDOVA- 14 TAMULEVICH 15 TENTATIVE RULING 16 Having read and considered the pleadings, declarations, and other evidence submitted in this matter, the 17 Court makes the following findings and orders: 18 A. Procedural History 19 1) The parties are Petitioner, Frank Cordova (Father) and Respondent, Chelsea Tamulevich 20 (Mother). The minor children subject to this proceeding is Valentino Tamulevich (DOB: 21 06/11/22) and Caspian Tamulevich (DOB: 06/26/21). 22 2) On May 30, 2024, the Juvenile Court issued a “Custody Order-Juvenile-Final Judgment” in 23 which Father was awarded sole legal and sole physical custody of the minor child, Valentino 24 Tamulevich and assigned Family Court case number FLD-24-397182 to the matter. 25 3) On June 14, 2024, the Juvenile Court issued a “Custody Order-Juvenile-Final Judgment” in 26 which Father was awarded sole legal and sole physical custody of the minor child, Caspian 27 Tamulevich and assigned Family Court case number FLD-24-397193 to the matter. 28 4) On May 21, 2026, Father filed a Request for Order seeking orders that: 29 a.
He be added as the legal Father on the minor children’s birth certificates; and
1 b. An order changing the minor children’s names. In particular, he is requesting the 2 following changes: 3 i. Caspian Anthony Tamulevich to Caspion Anthony Cordova-Tamulevich; and 4 ii. Valentino Dior Tamulevich to Valentino Dior Cordova-Tamulevich. 5 5) On July 1, 2026, Mother filed a Responsive Declaration, which has been read and considered by 6 the Court. 7 B. Findings and Order 8 1) This Court has jurisdiction to make child custody orders in this case under the Unform Child 9 Custody Jurisdiction and Enforcement Act.
A violation of this order may subject the party in 10 violation to civil or criminal penalties, or both. The country of habitual residence of the minor 11 child is the United States. 12 2) The Court sua sponte consolidates case number FLD-24-397193 into case number FLD-24- 13 397182 (lead case). All future filings shall be filed in the lead case. 14 3) Father’s request to be added as biological Father on the children’s birth certificates is 15 GRANTED. 16 4) The California Department of Public Health, Vital Records Unit is hereby authorized to add 17 Frank Cordova’s name, as biological Father, to the children’s birth certificates. 18 5) Father’s request for an order changing the minor children’s names is GRANTED.
The children’s 19 names shall be changed as follows: 20 a. Caspian Anthony Tamulevich is changed to Caspion Anthony Cordova-Tamulevich; and 21 b. Valentino Dior Tamulevich is changed to Valentino Dior Cordova-Tamulevich. 22 6) The Court will prepare the Findings and Order After Hearing. 23
